Athletic Eligibility
In order to be eligible to practice and compete in athletics in the School District of Jefferson, students must have the appropriate athletic items completed and on file with their school's main office for the current school year.

Scroll down to read more about each requirement and find the forms. It is the responsibility of the athlete to check with the main office for clearance verification. All of the requirements listed above must be completed in order to be given clearance for participation in Athletics. Feel free to stop in the office or call to check on eligibility status.

A student may not participate in interscholastic athletics (practice or competition) until the school has a WIAA Physical Clearance Form or Alternate Year Form that attests to the student’s physical fitness (as determined by a licensed physician).
The Physical Clearance Form must be completed by a doctor, signed by your parent or guardian and returned to the school's main office.
Physical Form Requirements for 2025-26 School Year:
- If your physical form is dated prior to April 1, 2024, you will need a new Physical Clearance Form completed.
- If your physical form is dated between April 1, 2024- March 31, 2025, it is good for 1 more school year with completion of the Alternate Year Form.
- If your physical form is dated between April 1, 2025- Present, it is good for 2 school years with completion of the Alternate Year Form for the second school year.

Athletic payments can be made in person in the school's main office or online through eFunds.
High School athletic fees are $50 per sport with a maximum of $100 per year per athlete.
3 Sport athletes should pay for their Fall and Winter Sports and the Spring Sports fee will be waived.
Middle School athletic fees are $20 per sport per athlete.
Concussion & Sudden Cardiac Arrest Information

All Student-Athletes must complete one Concussion Parent/Athlete Agreement Signature Form for each School Year. High School students will sign off on this requirement within the Athletic/ Co-Curricular form found below.
Some of our sports require an ImPACT Test to be performed prior to participation. The District Athletic Trainer will complete this with athletes when required.
Additional High School Eligibility Requirements
All students planning to participate in a sport or co-curricular at Jefferson High School must complete a Co-Curricular Registration Form each year.
This form includes:
- Parental/ Medical Consent
- Student Pledge (JHS Co-Curricular Handbook)
- Concussion, Head Injury, & Sudden Cardiac Arrest Acknowledgement/ Agreement
- Code of Conduct Signature (Code-of Conduct Meeting/ Handbook)
- WIAA Rules of Eligibility Sign-off

Code of Conduct Meeting
The mandatory high school athletic code of conduct meeting will be offered on two dates in the High School Commons:
- Wednesday, July 16, 2025 at 6:00pm
- Monday, July 21, 2025 at 6:00pm
All high school students planning to participate in a sport or co-curricular must attend one of the Code of Conduct meetings with a parent or guardian.
Athletic paperwork can be turned in at one of the meetings or to the high school main office before the athletic start date. Payments can be made online through eFunds.
