Student Immunizations

Student Immunization Law Requirements

Wisconsin State Immunization Law (s.252.04, Wis. Stats.) requires that all students through grade 12 meet a minimum number of required immunizations prior to school entrance. To ensure that our students are protected from vaccine-preventable diseases, the School District of Jefferson will exclude non-compliant students, grades preK-12, on the 30th day of school, each school year.

Medication at School

Medication should be administered at home whenever possible. If it is necessary for a student to take medication at school please follow the District's policy and procedure on medication administration. 

The Authorization to Administer Medication form, found in the right side menu, will need to be completed and turned into the school office before your child will be allowed to have medication at school. Please note that over-the-counter medication requires parents signature only and can't exceed the manufacturer's recommended dosage. Prescription medication requires both the prescribing provider's signature and the parent signature before medication will be administered at school.

Extended or Overnight Field Trips

ALL students must have a completed Overnight Field Trip Medical Release form, which can be found within our Health Forms.

If your student will be taking medication during the trip, you must also complete an Administering Medication to Students on Overnight Trips form.  Please note that students are not allowed to carry or self-administer controlled substances such as Ambien, Ritalin or Vicodin on the trip.  A staff member will be trained and assigned to do so.

  • Prescription medication:  Parent and medical provider signature is required.  The medication must come in the original container dispensed from the pharmacy. 
  • Non-prescription medications:  Parent signature is required.  The medication must be in the original container.  Students must not share medication with other students.
  • Please try to limit the amount of medication that is sent on the trip.

All required health forms must be completed and turned in to the School Nurse two weeks before the trip. If you have any questions regarding any of the health/medical forms, please contact the School Nurse.

Meningococcal Disease Information

According to section 118.07(3) of the state statutes, each school district must provide the parents/guardians of students enrolled in grade 6 to 12  in the district with information about meningococcal disease, including: (a) the causes and symptoms of the disease, (b) how it is spread, and (c) how to obtain additional information about the disease and the availability, effectiveness, and risks of vaccinations against the disease.
